PSYCHOANALYSIS IS A JORNEY

Psychoanalysis for a patient who has never undergone therapy, I believe this analogy is very appropriate:

 

“Imagine [analyst, talking to patient] that therapy is a long trip that we both will take together by car.

It’s a different journey, because we don’t know exactly where we’re going to end up;

the most important thing is the journey we will take together.

Who will drive the car?

You. Yes, it may seem strange that you, the patient, are the one guiding this process, but trust me: it's better this way.

Well, that means I, as a therapist, will be in the passenger seat, right?

But don't worry: I won't be sleeping. I'll act as a sort of co-pilot for you.

During the trip, your task is very simple: you just have to drive the car wherever you want and describe to me what you see ahead and around you.

No, we don't have a map. That's the beauty of this trip. There's no planned route. It's up to you to decide where we're going.

And me, what will I do?

As I said, I'll be a bit of a co-pilot. Every now and then, I'll ask you to turn around so we can go over certain places again and I'll draw your attention to certain things that you might have missed.

Trust me: as we make this journey, little by little all the suffering that made you come here will pass.

Maybe after a while you want to park the car, but you might also enjoy the trip so much that you don’t want to stop driving.”

PSYCHOANALYSIS

Psychoanalysis was created by Sigmund Freud in 1896. It is an analytical practice that has a clinical organization, but is not reduced to a technique, it is also a discipline that involves a system of thought and a modality of knowledge.

Our body expresses itself through symptoms, restrictions, repetitions and fears. Symptoms can be seen as a particular and unconscious way for the subject to organize their suffering.

We are often suffocated in a web of problems, people, stories, looks and speeches, to the point that we are unable to escape a repetitive cycle of failures and suffering. To undergo analysis is to participate in something absolutely unique in an impersonal and massified world and to have the possibility of escaping a frozen and absorbed way of listening that is characteristic of our time.

The psychoanalytic method is based on free association to access the unconscious, meaning that the patient speaks freely, which is why psychoanalysis became known as the cure through words.

The analyst does not guide the analysand's life and does not propose rules of action for achieving well-being, but we conduct a treatment so that the subject finds knowledge in the unconscious and can build another destiny for himself.

THERAPY SESSIONS

Psychotherapy sessions are a meeting between the therapist and the patient, via video calls and always in a protected and private environment.

In this meeting, the therapist/analyst listens to the patient/analysand's speech without judgment and only interrupts him/her when a reflection on what was said needs to be highlighted. During these sessions, the patient's initial complaints seem to diminish, until they become memories without any emotional suffering.

These sessions can occur once or more times a week, depending on the patient's needs and availability, and last approximately 50 minutes, at the therapist's discretion.

THE TREATMENT

The purpose of Psychoanalytic Psychotherapy is to help the patient talk about themselves, and from this speech, get in touch with content that is beyond their understanding (unconscious content), but which, however, influences their way of acting, thinking and feeling.

The role of the Therapist/Analyst in this case is that of a professional with a differentiated listening style, it is the role of listening to what is behind everything that the patient brings consciously, it is to listen to what is unconscious.

By interpreting the content brought up, the therapist/analyst will help you access your unconscious content, and thus contribute to a process of self-knowledge, enabling a greater understanding of some issues in your life and, with that, enabling a new way of being in the world. In addition, of course, to offering a welcoming space free from judgment.

Sessions are usually weekly, and may occur just once a week, or more than once, depending on the needs of each case and the patient's availability. The length of each session may vary depending on the professional, but on average, it usually lasts 50 minutes.

In the first meetings, initial interviews and some tests usually take place, such as: - Cross-Sectional Symptom Scale , which is an instrument that tracks mental health care indicators in 13 domains, namely: 1) depression; 2) anger; 3) mania; 4) anxiety; 5) somatic symptoms; 6) suicidal ideation; 7) psychosis; 8) sleep disorders; 9) repetitive thoughts and behaviors; 10) memory; 11) dissociation; 12) personality functioning; 13) substance use;

- Attention Deficit Hyperactivity Disorder Scale, the ADHD scale – Adolescent and Adult Version (ETDAH–AD, Benczik, 2013) developed for the Brazilian population, through statistical studies, demonstrated an excellent level of precision and reliability, has 69 items and evaluates five factors: 1) Inattention, 2) Impulsivity, 3) Emotional Aspects, 4) Self-regulation of Attention, Motivation and Action and 5) Hyperactivity.

Through the interview and tests, the patient will express the reasons that led him to be there and, together with the therapist/analyst, will understand what is the best path to follow.

Psychoanalytic therapy, in addition to promoting self-knowledge and improving emotional control, is used to discover childhood traumas, depression, anxiety disorders, sexual and relationship problems, among other internal issues, and treat them. In this way, the patient improves their quality of life and maintains their mental health.

The treatment can be combined with other techniques that complement and enhance the results, such as psychoaromatherapy and mindfulness techniques.

It is the psychoanalyst's responsibility to interpret the patient's speech and analyze it, seeking to find the traumatic origin and conduct the therapeutic process in order to minimize suffering.

Psychological problems that require medication should be treated by a psychiatrist. However, the psychoanalyst can identify possible emotional or personality disorders and refer the patient to the most appropriate care for their problem.

DEPRESSION

Depression presents, in addition to mood swings or irritability, anxiety and anguish, several signs and symptoms, which can be isolated or combined. The main symptoms of depression are:

  • irritability, anxiety and anguish;

  • discouragement, easy tiredness;

  • decrease or inability to feel joy and pleasure;

  • disinterest, lack of motivation and apathy;

  • feelings of fear, insecurity, hopelessness, despair and helplessness;

  • sensation, uselessness, ruin and failure;

  • decreased sexual performance;

  • insomnia or early morning awakening;

  • pain and other physical symptoms not explained by medical problems

ANXIETY

A racing heart, sweaty hands, and a lot of worry. These are typical signs of anxiety, a natural feeling when faced with certain important situations in life. However, some people feel anxious for no reason and so frequently that it interferes with their daily activities. This could mean that they have an anxiety disorder.

When anxiety becomes a medical problem, it can manifest itself in a number of ways:

· Generalized anxiety disorder;

· Panic disorder;.

· Social phobia;

· Specific phobia.

COUPLE THERAPY

A good marital relationship is not based on a magic formula that solves all problems with the snap of a finger. That is why a relationship can be quite complicated. At times, the couple finds themselves at a loss as to what to do to improve the relationship. In these cases, couples therapy could be a good option.

Couples therapy is a tool that helps with mediation and intervention in problem-solving.

As for values, this narrative is quite subjective, the value of a psychoanalysis session is only defined by the patient after understanding how much each session can add to the pain that brought him to therapy.

However, it is important to emphasize that the psychoanalyst's commitment is to truly improve the quality of life of his/her patients, where socioeconomic reality cannot be an obstacle to the recovery of mental health. Therefore, when entering the first consultation, a screening is carried out in order to verify the patient's real possibilities. In my opinion, mental health is above monetary values, therefore, we have a table with the maximum price, but I also have a table with a minimum price that will be adjusted according to the patient's financial reality, so that everyone can take care of themselves.
